To the comment on Canada, personally I had a Canadian physician as a child and the "wait" was not long at all. There are several parts to Medicare. What you pay in FICA taxes is for Medicare Part A. It pays for 80% of your hospital stay. You pay the rest out of pocket

To be covered for doctors visits and surgical costs, at age 65 you pay premiums for Medicare Part B. It pays up to 80% of those costs.

Medicaid Part C is an HMO for those who feel this fits their needs. It pays nearly 100% of all cost but treatment and choice of doctor is limited

The ones that don't have it are either too lazy to go out and apply for it or just don't want it. Plenty of programs in the US provide free or assisted health care, Medicare and Medicaid are two of them. I received 100% free healthcare till the age of 21, when I got a job and my employer provided it. My parents right now receive free healthcare and SS benefits, they are both over 65.
