I missed all three my first play through as well. Isn't that common? Two of them require you to be an idiot, and the other one requires you to be flat out stubborn. Interestingly though, I got the Pit Boss achievement on my own in a second play through, just because I wanted to see how Wheatley would react if I actually jumped into the pit. Got a surprise achievement out of it.

It's pretty easy to miss all three on a normal playthrough.

1: Why listen to GLaDOS when you're trying to escape her?

2: When you just want to get the test done, why would you stop and wait?
