They are one and the same. But Karas is the form that can only be used when Otoha is in his spirit form (like when he was in a coma). This time, he kinda projected his spirit out consciously, that's why you are seeing 2 Otohas.

Does it matter? Otaha is japanese but a white guy is voicing him but Does that matter? No it doesn't. And yurine isn't white either she's a cat spirit so we don't know what she is. But because this is an anime and the setting is in Shinjuku it can be inferred that she is japanese. I think some of you people are unconsciously racist if you have to point something like that out.
