1) The shot is mid shot so we don't see the ground at all. As the distance from subject is constant it is probably on a dolly with a circular track...It could be steadicam but I doubt it.
2) This is a cut all of its own. The other tracking shot was closer, this one is wider. So the cast only have to sit on the floor, below camera, then when the camera tracks to a certain point the 3rd AD will que the to stand up, and then que the women to walk into frame.
Well i am no filming technique literate too however .... If you observe it correctly .. the angle is high enough from the ground level for the cast to crip and take the place ... while the camera is still reolving around .. And when the camera is still somewhere in the left side .. the cast takes place and when the camera reaches to the point, they get up ...
And the person is actually NOT standing on the GROUND ... but its round revolving plate which is fixed on the groud and rotates in the opposite direction to the camera ...... hence you have that beautiful effect felt .....

I'm not very filming technique literate, but the shot at the end of the scene has the camera circling around Gandolfini:
1) How did they do that? 2:05
2) How did the cast just appear from the bottom of the frame without a cut? 2:42
