Christopher and Jonathan Nolan did a Really Great job with the Character and Origin "stories" of The Joker. Those of you who don't really follow the Batman Comics, The Joker has a couple of different origin stories and they took that idea and used it to show that the joker is truly "Unpredictable", and since heath did a great job as the joker he puts so much emotion in these scenes, like when he tells Rachel "SHE CAN'T THE SIGHT OF ME" you can never really tell when he is telling the truth.

His mind is so twisted that even he isn't sure about his past, so to avoid whatever traumatic event that turned him into this he changes his past every now and then to suit his needs. Heaths acting is amazing here, Michael Caine (Alfred) was supposed to say his lines as the joker walked into the room (You can see he looks as if he was about to say something at 8-11 seconds in) He'd never met Ledger before, so when Ledger arrived and performed he gave Caine such a fright he forgot his lines.
