It could have been both John's description of himself carrying amphetamines, as well as the dentist who slipped "L" into their drinks, substituting the name Robert, for the dentist's real name. Many rock musicians use clever ploys, ruses, and similar, in their lyrics, so people have to think about the meaning, and I would say that John Lennon was one of the ones who did this the most!

Dr. Robert was John and George's dentist. And they went for dinner with him and he slipped acid into their drinks. He advised John and George not to go outside but they were too high to care. So that was their fist experience of drugs. If you listen to the lyrics it's either about their dentist, a drug dealer or about drugs.
