I believe that it's about "that moment" a broad and general sense. That moment is what moves you in an amazing way. It might be an experience that you perceive as bad, it might be the best moment of you life, but ti's still "that moment" that he's referring to. That pivotal, inspiring, world shaping moment.

At least that's the way I take it.

I'm pretty certain it's about death and the situations encountered around death. Also has a lot to do with personal choices, personal feelings, and not so much where you go when you die but the experience instead and how we're all connected to death no matter what your religion is. Plenty of feelings that are individually interpreted though, so just listen to the music and let yourself decide how it makes you feel. That's what it's really about. :)
