Another quick one, Super Monaco GP on the Mega Drive/ Genesis. If you crash during the qualifying lap you're put in 15th Place with an initial position limit of 15th. Simply don't do anything when the light turns green at the start of the race, someone'll pass you and it's game over in about 3 seconds.

If I remember correctly, Space Quest 2 has a game over in less than a second. You can walk off the screen and into space for eternity if you just take two steps to the right. To accomplish this feat, set the game to its "fastest" speed setting and you can do it.

Technically, in the flight sim, IL-2 Sturmovik. If you start a mission with a bombload, and have to take off from a runway. If you release the bomb(s) right when you start, you'll blow yourself up instantly. I don't know how fast: haven't counted, but it does say "GAME OVER" in big letters when your pilot gets killed, so maybe that counts >3
