its an actual medical thing, it's called Disappearing Twins Syndrome. its when the mother's body isn't capable of supporting multiples and so it sacrifices one of them in order to keep at least one living, because otherwise both the twins would die. basically the writer is taking that medical condition and making it into a horror film by suggesting that the twin that "disappears" (dies) could come to haunt the one that lived. totally unrealistic, but makes for a great horror film!
