$10 for sunglasses - May or may not be worth it. I've seen sunglasses on the street that I'd pay $10 and others that I wouldn't. From what I've read, the quality is decent if you want a cheap pair of shades.

The only way to only spend $45 on shipping is if the whole original price for however many glasses was $500, and that is before the "discount". So if you take into account the "discount" then you probably would spend about $200 of the card for about 2-3 pairs of glasses. Well, unless you got undiscounted ones.
As for the actual check-out, there is another promotional offer for them to send you 2 pairs of sunglasses for free each month (except for $20 shipping and handling) for you to make comments about the design. The box is checked automatically, so if you're not interested, uncheck it.
