There is an elavation adjuster, so they opened up the hole while he was covered under the blanket and he just jumped in there. He was gonna do something similar like that in This Is It tour but instead he decided to jump out then go in.

@Kelbert88 ehm, this is a dvd release. Crowd noise is almost entirely removed in all of his concerts. In this case, they completely removed it and replaced it with cheers from various concerts thoughout the Dangerous tour. The crowd was probably awesome.
