looks like he went through the vein, no venous return after the intial flashback even when the tourniquet was still on.

Going through the vein means "Blowing the vein" altho if he did Blow the vein you would definitely know, even in the video.

its all about technique.. you got to find out how you feel most comfortable doing IVs.then it becomes 2nd nature. Repitition repitition repitition

he would have done it if the client was lying supine with his arm stretched or if he didn't insert too close on the antecubital fossa.

Not trying to be mean, but that is actually a 18gauge catheter. Standard color for 18G is green, if it were a 16 it would be gray.
