I've got this game for my Megadrive and I think it's one of the best! I can only get to level 3 or so before I start losing but it's alot of fun.

Left Button - Left Flipper

B - Tilt

C - Right Flipper

Goodness knows how you managed to get one button pressing both B and C at once :)
