The anime is called 'Boku no Pico' (well actually it's just an OVA)

It's about a dude named Tamotsu who works at a cafe and one day he meets a boy named Pico that looks like a girl so he instantly develops a sexual relation with him thinking he's a girl. Then ensues a creepy romance between them.

It's one of the most disturbing animes I've ever seen, it's like anime's equivalent to lemonparty O_O
