ok for all the questions, the ex girlfriend prob died by being hit by a car and the current girlfriend is in the mental hospital because she's haunted by the ex. The ghost is the ex.

The reason they made the music video freaky but the song happy is because it's a method used to bring more interest. You know in scary movies, rite during the scary part, they play a happy children's song, like ring around the rosie, to bring more intensity into it.. got it?
