I lived in Knoxville, Tennessee (near the Appalachians) for 14 years and I heard the Appalachian speech even there. People said 'flare' for flower, 'aire' for hour, etc. It was an education!
We see that all the time when an American news team does an article in the UK and they interview the local people. Often they will put in subtitles where as to me, they are not necessary. Depends on your background.

yup. My dad's family is from West Pennsylvania, but I don't have the accent, and I understood it perfectly.
