if the club didn't collapse over his shoulder turn, he could release thoroughly on the ball. the result was from turning way too open before the club returned to the ball, resulting in a loss of coiled power and 'toeing' the ball off the tip of the driver. keeping that lead arm straight at the height of your back swing then leading with the arms, keeping the lead leg planted, should help a lot.

also, back swing is a little flat. raise those arms a bit. comin in too low
