All you are hearing including vocals (obviously) are from his own mind, no one else works with him unless it the producing and he even does a large majority of that. It might just be the remastered video. I downloaded an "album" of the Woodstock performance that sounded a lot more raw, and at that part you can totally hear his guitar get noisy when he falls.

It's definitely noisy. His distortion effect for this song is uber compressed and bright. You can hear that when he's starting to trip chaos is coming out of the guitar and then he falls and it just keeps ringing. The sound setup is also an important variable in any performance so it may not sound like it sounded at the actual performance.
