Really, all this test is doing is putting him face to face with the consequences of his choices. It's easier to choose when you don't have to watch them die. His hands get pierced because it's supposed to be symbolic of the "blood on his hands".

It angers him because Him and people like him (cancer patients) would do anything to have that chance again, and the ones that do have it are throwing it away like garbage. I don't condone Jigsaw's work, but I understand why.
