you can follow the "flailing" connected to the valve--not necessarily true with vegetation. Another feature is the "flailing tubular feature" of the ruptured papilary muscle. Hyperechogenicity of vegetation is to also to be expected compared to a more hypoechoic muscle.
Chordae rupture on its own may not necessarily produce significant MR and would appear as a filamentous structure, the differnece with this clip is (upon pausing the cineloops) is the display of a significant mass (pap muscle)connected to the end of the chordae and subsequent industrial strength mitral regurgitation

How do you distinguish ruptured papillary muscle from vegetation?

look to see if it is the same texture/pattern as the myocardium. Vegetations typically have a brighter looking coloring from my experience.
