Both parties are clearly at fault however as the controller should absolutely not have cleared another aircraft to take off with an aircraft reporting that it was at the wrong place on the field, especially given the crew's insistence that they were close to the active runway.

I'm an ATC and I find unbelievable all this,expecially how she shuts up the UN1448 crew, during a position report, to clear the USAir to takeoff (a takeoff clearance with a strayed aircraft on the maneuvering area is out of imagination to me);in a very similar scenario in October 2001 in LIML airport occurred 118 civil victims. May they rest in peace, and may a huge thanks be given to the USAir flight crew, who showed the real professional and judgment that should always be put in this job.
This is amazing, because they're saying they're on a runway now and somebody took off over them a minute ago, but she still doesn't get it. The United Airlines crew should have been paying attention during taxi, but getting lost (and perhaps at an airport they haven't flown to before) can happen and happen easily.
However, there is no excuse for what the ATC did here. I am glad that the UA pilot advised ATC about being on an active runway and that USAir held short of the runway and refused to takeoff until the mess was sorted out.
Also, clearly UA was on an active runway BECAUSE you could hear the Fedex plane takeoff over ATC.

That USair crew saved a plane load of folks by following along. Can you imagine if this scenario had played out in Mexico where they're speaking spanish to half of the traffic?

Ugh, it was so creepy when that FedEx 1662 was taking off. I don't know why the ATC cleared the number 2 when United said "somebody just took off". That's how accidents happen.
Way to go for the US Air Capt not blindly accepting a bad clearance. I can understand getting lost in the fog. Can't understand a controller clearing take-offs without knowing where everyone is on the airport. Controllers can blow it, just like pilots... don't put all your confidence in them.
The controller should have listned to the united pilot when he said he was on an active runway, that should have alerted her that something was wrong. If it wasn't for the US pilot saying he wont take off there could have been a serious accident.

5L-23R was decomissioned and 5R-23L is simply now 5-23 since this incident. That should help prevent this from happening again.

This is amazing, because they're saying they're on a runway now and somebody took off over them a minute ago, but she still doesn't get it. It doesn't sound like they were on the runway when the first plane took off, but if they were sitting there and the second one did, and that plane used enough of the runway, it could have been a collision! I hope the controller paid more attention to what pilots were telling her in the future.

Its a mainline a319 from ord. All UAX flights begin with 5000 or greater. PVD UAX operator is Gojet and maybe Trans States.
