Ive had problems with the O-rings on the return lines, where the plastic piece fits over the injector but thats about it. The low sulfer fuel is hell in those old rubber lines.
Are you sure it just doesnt have a bad glow plug or two? These engines will shake like hell for a few secs till the cold cylinder has time to warm up.
It does have a couple dead plugs, but it has a leaky injector, leaks when the engine is off and causes pre-igntion, and does a super loud knock untill the extra fuel is burned.

When i worked as a bundle hauler for the newspaper we used these GMC Vandura's with a 6.2L, or 6.5L Diesels. When they were cold started they were very shaky, and hesitant.

No, it has old and dirty injectors the fuel is not getting to the engine well when it's cold, it only does it for a few seconds then stops, usually I give a bit of gas and it stops right away but I hate revving a cold engine.

Starts better than mine does on a warm day lol. Now that ive started running it everyday its better now. I think some fresh fuel did it good.
