Breaks in the icing are caused by little air pockets or uneven pressure. Even with 25 years' experience it's still hard to get all the air pockets out of icing when you're making it. Pressure control is something that comes through practice.
The only critique I have is the messy dot on the 'i' in 'birthday'. They should be clean. Yes, she went through it quickly, but if you're working in a professional bakery you MUST work fast to get as many cakes done as you can, especially if you have lots of orders. Her techniques were right on, but it isn't enough to watch it. 90% of all decorating is practice, and you don't want to start out on the actual cake. Practice boards are great beginner's tools.

Wow, I am a southerner and I was always taught not to say anything if you have nothing nice to say. Guess some people were never taught that, and do not care if they hurt someones feelings. So so sad!!
