Pressure might have built up inside the engine, containing the dangerous mixture still not burning... and then after some heating up the whole containing which could not escape out of the nozzle, caused this enormous explosion.

Where in the video did you hear that it was an easily visible crack? Perhaps the crack was formed on the inside of the casing, making it virtually impossible to see by ground crews.

I'm not sure of the configuration of this particular rocket, but I wouldn't read too much into the fact that some didn't appear to light up at launch.
