The gears are not needed, they are already on the bike. This is the beauty of hub motors, they do not put the power through an inefficent gear system but straight to the road, how much more efficient can you get?
but in some situations, you need torque, and in others you want speed. efficiency is a non issue, almost all adult bikes have gears. You can get electric bikes that utalise the existing gear system.
