The text is more than vague, there could be two different ways to interpret it: the main difference is weather it's about the girlfriend or the mother. in the refrain he's singing "mother said it all", so i think it's directed at his girlfriend.

but you can definitely say that he's ashamed because he left his girlfriend/mother when she had suicidal thougts, possibly after he hurt her. according to the text she's already dead and he cant let her go.
