Sometimes they slap them for real but it isn't a hard slap, they add in whats called foley sound effect. For example they record the slap on camera, then after they re-create the noise by, for example, clapping their hands once really hard, then dub it over the video clip. It IS quite effective in makeing it look like the girl really smacked him hard.
