When you use an AoE attack, you will get attacked by a mob (or mobs in some case), named "The Unseen". They are working as ghosts, but are yellow in thier appearence, so they won't attack you unless you attack them first.

These can only be seen if you are dead, or are using a spell that allows you to become a shadow (so you will enter the shadow realm pr. auto. Else, they are invisible to the eye. Its nothing else :)

There was a doll back in vanilia in one of these "ghost" houses in the ghost town. However, it got removed when patch 1.4 came out, (i believe that it was the blackwing lair raid that came out), but it did not get announced since it was not official. It was an NPC, friendly of course, who would look with evil eyes at you in that room no matter where you moved. But now, its only the ghosts, i know this from testing it :)
