The # just means that the D note is sharp, if you have a chromatic tuner then you should be able to get it easily, if not play the 4th string D on the first fret, that will be D sharp, then match your First string D to that note, but remember there will be an octave difference in them but you should still be able to hear when they are Harmonic

Tune it up one half step. Hold the B down on the fourth fret. That's the note you want on the top string.
