Actually, the reason you can't hear the crowd overwhelming him is because this show is extremely well mic'd, so the crowd's sound does not overpower his performance. Many stand up shows aren't as well mic'd, but HBO specials are usually made much more professionally.

Something has to be really funny to get American to laugh, they won't just laugh to appreciate the joke, as the British do. Instead, when there is a good joke but they don't feel it's tickling the funny bone, they simply applause in appreciation. So they do get it, they understand the point, they just react to it differently. You can see a similar pattern in George Carlin's stand ups too.
