in what period of time does something like this get to that size? and what are the dark spots? if you cut it open, whats in there? and why is it in his head?

I don't know how long it takes, but it's probably in there due to head trauma. From my understanding of abcesses it's your body's way of repairing itself. Like cancer your body can over produce when attempting to repair. The body can't absorb that much material, and the abcess solidifies, and becomes permanent until surgically removed.

I'm no expert though.
