I'm playing Sherlock Ho rite here, and here's my deduction:

The longer you leave Eileen alone the bloodier, creepier, and generally more fucked up she gets- and that results in the "bad" ending...?

@indigoXmoonlight Actually, I'm fairly sure it happens gradually, or is based on how much damage she takes. Since Snake has barely let anything touch her at this point, I'm fairly sure it's both. I saw a different playthrough where the guy playing didn't give a flying dick about Eileen, and she was completely red by the end of the Forest level. No matter what, Eileen will make it to the end of the game.

Also, the ending you get is mostly determined by the final boss. You'll see what I mean.
