When you create icon from the 3D model, there is a window, where you pick icon sizes to create. You can pick sizes from 1x1 to 256x256 there.
When defining the view point and lights, you can change size of the picture created from the 3D model by the left size of the window. The size here should by about 4x bigger than the icon size you would eventually create in the end.

RealWorld Cursor Editor is for cursors only and
RealWorld Icon Editor is for icons only

The sequence is:
1. 3D model (.u3d file)
click Operation->Render to image
2. Snapshot definition (.r3i file)
click Operaiton->Create Icon from snapshot
3. icon
click File->Save
