there are several different types of ulcers. the one that max seems to have is either a gastric ulcer, which is a small hole that forms in the stomach, or it could also be a peptic ulcer, which is a hole that forms in the small intestine.

By pretending it was in America, but not mentioning any other place other than Willow Creek. They basically stuck to the books story lines, though changing it here and there to suite the program. As to why they chose to film in Australia I'm not sure as to the why only that they did.
