I wish I had better advice. I have fired thousands of rounds through mine. No break in period. No malfunctions. Limp wrist, firm wrist, no wrist. It just shoots one ragged hole.
I have had 1 failure to feed in the thousands of rounds in my usp .40. I did a light polish the feed ramp and chamber with polishing compount and a cotton wheel in a cordless drill... the upper hood of the chamber was slightly scuffed up.. I have had absolutely no issues since... feeds everything and runs perfect.
I've shot all sorts of .45 ACP FMJ out of my tactical (approximately 1500 rounds), and it's only jammed on me once. The one failure was the extractor did not catch the rim of the casing, probably caused by an out of spec reload. Although I keep my tactical in pristine condition, I don't think cleaning, or a lack thereof, is the cause of this users failures to feed. Something must be out of spec. It almost appears as though the top of the casing is catching on the feed ramp or something weird.

I would check your recoil spring and your ejector.

Try cleaning it. I've had a USP 45 fullsized for over 13 years and it has been totally reliable.
Don't blame the pistol for errors made by the operator.

Have you tried some better ammo? Rem/UMC is pretty much the bottom of the barrel for factory FMJ IMO.
I use it all the time because I'm cheap. :D
