According to the Star Trek Enterprise episodes "Affliction" and "Divergence" Dr. Arik Soong created the Augments and the Klingons later tried to use the Augment DNA to created better Klingon warriors. Instead, they created Klingon/Human hybrids that we would then later see in Star Trek: TOS. Supposedly, the Human DNA degraded over generations and the Klingons regained their natural appearances. ;)
Some of these are omissions or generalizations rather than paradoxes though.
-When Kirk says "the hardest substance known in the universe" about diamond, he most certainly omits to add "natural ... outside of singularities and star cores".
-"Klingons don't take prisoners" and "Vulcans don't touch food with their hands" are generalizations.

Stereotypes are very often correct, but it is in the minority of instances where they are incorrect that they are loudly proclaimed to be wrong. This is obviously because we don't wish to be put in "boxes," so when we find that that has been done we fight against us. It is a human assumption that if we can define something we have power over it.
Humans have eyes. Humans have toes. Humans have ears. Humans have arms. Humans have a tongue. Humans can make conscious decisions. Humans are self-interested. Humans have the ability to communicate. Native africans have darker skin than native europeans. Pre-pubescant humans have hair on their head. There, 10 stereotypes that are true 95% of the time. If we didn't make these assumptions we would be making calculations all the time on what is and isn't true. Society couldn't exist without them

I also fail to see how "Humans have eyes" is an assumption, when every single human being has eyes. They may not function correctly, but they do exist. Just because the actress knew it was a continuity error doesn't negate the fact that it ended up appearing on screen and thereby is canon. Although one could say that it was character development, part of T'Pol becoming more "human" and emotional than other Vulcans over the course of Enterprise. Notice how Archer gives her a weird look when she eats it too, as if he's surprised.
the part with Vulcan can be explained by saying that Vulcan itself is a moon of a bigger planet (the one seen in the sky) which also has other moons also visible from vulcan. so it may seem like Vulcan has moons but actually it doesn't.
Andoria was also always thought to be a planet but it turned out it was a moon of a gas giant (Andor)
The image from this video is from Wrath of Khan. The director kinda acknowledges the break in Star Trek continuity. In the "director's cut" release in of the DVD in 2002, the night time moon scene was replaced with a daytime desert scene.
This was a very amusing video showing the errors, both based on science and continuity, that are persent everywhere in Star Trek. I can't wait until we add JJ Abrams' movie to this mix of mistakes. It'll be more fun than a drinking game!

Having Kurge want prisoners was purposefully done due to having Kirk claim that they don't take prisoners. Chekov not being in Space Seed never bothered me because it'd be presumptuous to assume that he was not one of 400 crew members on board the ship. Vulcan's don't eat with their hands, but Vulcan's also don't lie. You could do a 10 minute segment on just that one subject.
