I think it's about a girl dating a guy who she likes, but she's too high all the time to hide the fact that she's also cheating. Everyone knows her ways but she doesn't want it to get around to her boyfriend cause she doesnt want to choose between her lifestyle or him. I think my favourite part about this song is the comments. Everyone gets a different meaning from the song. My take on the song:

The girl gets drunk on her birthday and sleeps with some guy in her room with the window open. Everyone sees. So she and the guy get into a relationship and he starts beating.

The first time the chorus starts "twist" is sexual, as in the twisting of limbs.

The other times the chorus starts, "twist" is trying to writhe away, as in twisting limbs away from someone to avoid a slap or something.

It's a sad song when you think about it...
