Clever, he swapped it round. If you freeze it as he kills em, you see the doctor standing to the left, if you then rewatch from the beginning you see them running to a fallen man.

Exactly, that's why Peter Davison was able to regenerate into Collin Baker, the disease took long enough for his brain to realise that it was in danger and start the regenerative process. Think about it, if all the time lords could regenerate 12 times in the Time War then it would take forever and the Daleks would just get bored and destroy another civillisation
