Im an aircraft mechanic but iv never worked on dash-8s but from what i hear if your not careful, you can misalign the wheel during installation and it will come off.
Funny thing is Southwest had a tire FIRE on landing on the SAME DAY!!!!!, but never made the headlines. Colgan ended up at the gate while southwest had to evacuate . Funny how the media works
Most aircraft manufacturers have a specially designed axle nut that does not allow the wheel to seperate even when bearing failure occurs. I can't believe this aircraft does not have this as long as it has been around.
I have seen about 5 planes have similar problems to this. It is due to bearing failure but that can be caused by several idfferent causes. A bad batch of grease, someone new in the tire shop not properly lubricating the bearings during tire buid, I have even seen instances where the grease was contaminated by hydraulic fluid caused the bearing to overheat and fail. Mixing of different types of grease is another possible cause.
Bearing failure. I believe no one is at fault, things will break no matter how reliable they might be. In this case, the bearings ceased causing them to fail. The mechanic that replaced the main wheel assembly was not found at fault in this case. I know because I work for Colgan and I personally work on the Q400's. People here mention of cotter pins and other things, but truth is the cotter pins were found were they were supposed to be, conclusion: mechanical failure.
this is thrown way out of proportion- most of the incidents involing these accidents have been proven to be the result of the lack of maitanence the airline companies performed on the aircraft-its not the aircraft manufaturer or the aircraft its self.
I believe the Pilot did a great job keeping the trust load to the outside wheel. .He was hopeing it would hold and it apears it did. Who ever performend the final inspection will be at fault asumeing no parts actually failed. Maybe a simple cotter pin . The investagation will find out. All was just fine from what I see so far. No more Drama
Well, if i was the person with the camera I would be terrified no matter what many of you say.

I loved flying the SAS dash 8 q400 just loved it until they band it in Aalborg and the rest of Denmark. I am from Aalborg,Denmark and i was actully at the airport at the time i could hear the sirens i was getting ready to board a Cimber plane to copenhagen.my flight was delayed 5 hours before i could take off.
The Dash-8 is a great aircraft, i have more than 10000 flight time and more than 6000 hrs flying one for JAZZ in Canada... in bad weather or short runways it' the best, it's built solid and it is built for Canadian weather...I would not fly an other aircraft...
In my opinion SAS used their incidents to blame the aircraft rather than poor maintenance. these regional aircraft takeoff and land more than larger jet aircraft like the 737 A318 & up so if they are not maintained properly they are more likely going to have issues like this Q400 are great aircraft and the fact that these landing from landing gear problems are an example of the aircraft and highlights the importance of maintenance in high demand aircraft.
The wheel fell off because of faulty wheel bearings that sheared off the axle, and the brake fell of because the wheel holds it in place. It really not a big deal. Thats why there is 2 wheels.
The axle didn't shear. Look closely, it's still on. A castellated nut wasn't safety-wired/tabbed & thus spun off on touchdown when the wheel spun, losing wheel, bearings & brake assembly. It's only a slightly-bigger deal since now the brakes are nonfunctional; see that leaking fluid? Plus, only one tire takes a much greater load. If you saw the full video, you'd see lots of leaking brake fluid & a nearly-flat solo tire. The plane crept into the gate at 2mph.
"Whew, I just barely got that plane out of the hangar on schedule for next flight after replacing all tires. Say, I wonder where these 6 leftover nuts were supposed to go? Oh well, must not be important, I'll just chuck 'em."

I'll trust the Dash 8 any day over certain other pieces of 'junk'. The Canadians build these things like tanks. Aside from the landing gear issues (which I believe have been maintenance issues more than anything) the Dash 8 is a reliable plane. Sure things do break on them a bit such as the NVS (noise cancellation system) but at the end of the day this plane will withstand a helluva lot...and get you there safely.
They are built by the same company, and I'd trust any airplane from them (Bombardier) over a lot. I think that the Dash 8's are better built than anything else in their catagory.
it always sad that the general public really never understand. I fly for a living and also have a my A&P (airframe and powerplant license to work on aircraft). As always it easy to blame the pilot or the mech. Just as in your car parts break.. Learn before you talk...
@TensaZ, what do expect by the maintenance practices. The current startubg pay for airline mechanics is about $15.00 per hour compared to being $20.00 an hour back in 1999. The airlines are cutting cost by paying mechanics less and raising airfares so dont be surprised for the increase in accidents.
My sister use to date an aircraft mechanic, he saw this video and immediately said they probably forgot to torque properly the Wheel and brake assy and install the retaining nuts. He also said you should see the kind of things that happens in aviation when you forget to install even a simply pin cotter.
If you torque down the retaining nut then wheel would not turn, it would bind on the bearings. There is a cotter pin that should be inserted into the crowned retaining nut. I've seen it forgotten before.
I pulled the cap off my wheel to add air to the tires and noticed that the shop that did my annual had not installed the cotter pin. Could have been an expensive situation.
actually this kind thing is not that uncommon, just e few months ago a Dutch Antilles Express ATR 42 tire flew off from the nose gear while landing in Curacao.
would have been better without the music. And there's nothing wrong with this aircraft. When an airline has poor maintence practices what else do you expect.
This person obviously has a problem with the Q400. SAS was found to be negligent in the maintenace of their aircraft which is why they had problems with the landing gear, and the same applies to Colgan Air, who are not only lousy at maintenance obviously but also at training their pilots how to fly the damn things. The NTSB pointed to pilot error in the Colgan crash because of inadequate training. Look it up, it's a matter of public record. Kindly get your facts right before you post, please.
@richardds9 I believe the crashes that happenned was due to human failure, or sucessive 'small' hard landings, wich are very common in Dash8...But how do you explain that some of the Q402NG delivered to Sata Air Açores, with something like 10 flight hours(including delivery flight), after it's delivery flight had to be put in the hangar for repair...(it was told me by an engineer) They are great machine with outstanding power, but doesn't seems to be very reliable...

@richardds9 I went to college and flight school with the FO in that accident. She and I studied a few NTSB cases covering the very topic of "sterile cockpit." Both pilots knew better... sometimes shit just catches up with you.

This is a maintenance problem. Some poor guy on a monday morning hung over forgot to put the cotter pin in after a tire change, and which over a period of time the bolt holding the wheel and braking system in place simply loosened and as you can see, this is what happens just from a simple pin.
Landing gear like this don't use cotter pins in that way. They use two bolts to hold the axle nut on. I think this was more likely a part failure than a faulty installation.
It seems that ProzrachnyTeatr would rather trust his fate to the legions of Ilyushins and Tupolev "gravity provers" rather than the 2nd and 3rd generation old aircraft the entire Eastern bloc flocked to buy up to replace their fleets full of "Isaac Newton Specials".

It's already been shown that the Buffalo crash had nothing to do with icing. It was purely pilot error. The pilots allowed the plane to enter into a stall, and both the pilot and first officer responded incorrectly to the situation causing the crash.
The Q-400 has had a long history of mechanical problems. Most of them having to do with the landing gear. SAS completely removed the aircraft from their fleet due to all the problems they have experienced

Bombardier and Goodrich really needs to redesign the landing gear! The Q400 airplane itself is actually pretty good, but the landing gear is the Achilles Heel. I see a future of either no Q400s in commercial service, or lots of Q400s with nearly indestructible super-reliable landing gear.
Isn't the operation of portable electronic devices during that phase of flight a violation of Federal Aviation Regulations? I dont think that camera should have been on and taping.

Yup the passenger violated an FAR... but getting this piece of footage transcends that completely because it lets the NTSB and manufacturers clearly identify what transpired. As a pilot I'm kinda glad he/she did film it.
Did you see that MSNBC and other networks are showing the wheel falling off and the other (different) plane collapsing as the same plane? How is that possible? One plane collapses and one doesn't. Plus, how could the plane that collapsed be back in service so fast? Media manipulation again!!

this looks like a maintenance error to me. Prob caused by a wrong torq vallue of the mainwheel nut and not locking it right. But nice cam shot, you can even see the wheel, spacers and even the brake all taking off. Ans still rolling.....fail safe....:-)
technical glitches? if i remember right, every accident/incident with the q400 was pilot error (Colgan 3407) or improper maintenance procedures (SAS)... Some people, including the media, give a perfectly good plane a bad name because the pilot pulled up in a stall, or someone put the gear assembly together wrong... most aircraft can lose a single wheel and still stay upright. not a big issue safety wise. Airplane are designed to take a lot of abuse on landing.

actually this kind thing is not that uncommon, just e few months ago a Dutch Antilles Express ATR 42 tire flew off from the nose gear while landing in Curacao.

This person obviously has a problem with the Q400. SAS was found to be negligent in the maintenace of their aircraft which is why they had problems with the landing gear, and the same applies to Colgan Air, who are not only lousy at maintenance obviously but also at training their pilots how to fly the damn things. The NTSB pointed to pilot error in the Colgan crash because of inadequate training. Look it up, it's a matter of public record. Kindly get your facts right before you post, please.

@richardds9 I believe the crashes that happenned was due to human failure, or sucessive 'small' hard landings, wich are very common in Dash8...But how do you explain that some of the Q402NG delivered to Sata Air Açores, with something like 10 flight hours(including delivery flight), after it's delivery flight had to be put in the hangar for repair...(it was told me by an engineer) They are great machine with outstanding power, but doesn't seems to be very reliable...

@richardds9 aye i'll give you pilots but that wheel flying off was a bering faliure that axle could not contain bc both of the bearings falied on that and pretty soon goodrich is gonna come out with an AD for that oh an by the way that tire was on that airplane for a month without a glitch just so you know.......

@richardds9 I went to college and flight school with the FO in that accident. She and I studied a few NTSB cases covering the very topic of "sterile cockpit." Both pilots knew better... sometimes shit just catches up with you.

This incident probably has nothing to do with the Q400s design, just a mechanical failure, as Engineers/Technicians/Pilots/mechanics have said.
Interesting to see all the bits fall off though, must of been a bit of a shock

This is a maintenance problem. Some poor guy on a monday morning hung over forgot to put the cotter pin in after a tire change, and which over a period of time the bolt holding the wheel and braking system in place simply loosened and as you can see, this is what happens just from a simple pin.

Landing gear like this don't use cotter pins in that way. They use two bolts to hold the axle nut on. I think this was more likely a part failure than a faulty installation.

It's already been shown that the Buffalo crash had nothing to do with icing. It was purely pilot error. The pilots allowed the plane to enter into a stall, and both the pilot and first officer responded incorrectly to the situation causing the crash.
The Q-400 has had a long history of mechanical problems. Most of them having to do with the landing gear. SAS completely removed the aircraft from their fleet due to all the problems they have experienced

compare dash incidents with any other manufacturer and tell me who has more incidents !!!! a tire flies off on landing and the plane is a piece of crap hahaha, everyone lived, proper maintanace and pilots doing there checks before take off helps to notice small problems like a loose tire !!!! plame the airline not the plane ;)

Bombardier and Goodrich really needs to redesign the landing gear! The Q400 airplane itself is actually pretty good, but the landing gear is the Achilles Heel. I see a future of either no Q400s in commercial service, or lots of Q400s with nearly indestructible super-reliable landing gear.

Yup the passenger violated an FAR... but getting this piece of footage transcends that completely because it lets the NTSB and manufacturers clearly identify what transpired. As a pilot I'm kinda glad he/she did film it.

Did you see that MSNBC and other networks are showing the wheel falling off and the other (different) plane collapsing as the same plane? How is that possible? One plane collapses and one doesn't. Plus, how could the plane that collapsed be back in service so fast? Media manipulation again!!

technical glitches? if i remember right, every accident/incident with the q400 was pilot error (Colgan 3407) or improper maintenance procedures (SAS)... Some people, including the media, give a perfectly good plane a bad name because the pilot pulled up in a stall, or someone put the gear assembly together wrong... I dont see how this is the plane's fault... Poor Description
