The reason why he does that is because to make it look like he had scars Heath Ledger had to wear mouth prosthetics and like anyone would do he developed a habit of feeling it with his tongue. Someone working on the movie noticed this and decided it would add something to the character.

jazzstone has a point but not completely correct. guarantee you the licking is because he has raw skin/scar tissue on the inside of his cheeks and on his lower lip. think about it...like you said you lick your lips when theyre chapped. but you also lick your lips a ton when you know you have a split lip or a cut or some shit.
