Elizabeth and her mom lived in a DIFFERENT city but they moved recently to be closer to Nikki who spent half her life in the asylum in a different city. That's why elizabeth is new to the town while Nikki's been there for half her life. Blake was ALSO in the SAME asylum as Nikki we he was YOUNGER and they were friends, but Blake got better and was released while Nikki still stayed in the asylum.

And the meds are made to keep brain functions normal, that way they can work towards helping them. Those places actually do help quite alot of people...
