Its all about the first minute of the song!!This is not a mod. All I did was recorded myself playing Dr Mario and I put a midi version of Perfect Drug that I modified through a midi program. This midi might sound all the same to you but this is completely different to me in my ears. Its all about the triangle waves and pulse waves.

I've seen more extensive ROM mods before. It's certainly possible to create a hacked-up version of the game with this music.

Lack of other sound effects does not necessarily indicate that it must be overlaid music.
