@paolagzzgzz most likely because spread of cancer. Also if they asked the patient involving risks of retaining the nipple might cause regress of the tumor. This is also the reason for why they dissect the auxillary lymph nodes.

@paolagzzgzz Ductal carcinoma is highly metastatic and was likely to have spread proximal (close) the the areola and nipple, like it had to the axilla lymph node that was mentioned in the video.
