I have the same problem, I don't stutter in single player but in multiplayer I stutter like crazy, exactly like you were, did you ever find out how to fix it?
My BO worked fine before i upgraded to win7 from vista... now its almost exactly the same as yours. i had all settings maxed with 0 problems, after upgrade everything just went crap! so im guessing its something to do with some driver, gonna try reinstall video/audio and see how it goes. will keep you posted.
I have the same issue but my game runs on the lowest settings possible and this is what it looks like, 800x600 rez, anti-aliasing off shadows off sync every frame off tiny corpses soften smoke edges off etc... Im on a Gateway windows 7 with a 10mbps upload and 15 download, so it must be my video card or somthing, if anyone could please help me with my problem reply to this post with a solution.
I am having the same problem. Funny enough, before the 12/14 patch I could run the game maxed out with no issue. Anyway, I found the issue to be 'Texture Detail'. Lower your 'Texture Detail' to 'High'. Extra makes mine stutter. When I have it on High with everything else maxed out it runs just fine!
@QuiDxRazer I didn't want to affect the actual look of the game. I don't wan to disable sound since that will affect the game even more. Maybe I'll give it a try, it's wrong to be stubborn on some things, but I am still drawing the line with sound, heh :P.
@kickpush33 This will give me a lot of screen tearing though since I have a 60hz monitor. Do you think if I keep it forced in my driver control panel this will be okay?
@fac7orx Yea, im not an expert but i think it should be fine. i have a 20" 60hz monitor and taking off sync every frame helped out. also QuiDxRazer is right too, taking off how some of the ways the game looks will improve performance. also make your aspect ratio 16:9 instead of auto, and anti-aliasing to 16x. Thats what I have, but yours may be different. just mess with the settings until it shows improvement.
@kickpush33 I already had my aa set to 16x before and it didn't help too much. However sync every frame helped a bit. I also tried setting shadow to off, but that did nothing.
I get over 60fps all the time but only when shadows are turned off, i used to stutter like this but it was fixed with changing to 44khz in cfg and realtek settings. Also I heard XP runs it better for some strange reason.. good luck I hope you find a fix soon
@detjkimbell I'll give shadows a shot, but I already tried matching the sound. I'm not going to install xp though, so that one is out of the question atm.
I can confirm this, Black ops run ALOT BETTER on WinXP. Even a dual core can run this at 60fps constant without any stuttering as long as you're on XP.

After tons of stuff that didn't work, I found this which seems to have worked ^_^
Go into your cfg. file (for both multi and singleplayer) and open it in note pad
press ctrl + F and search for "seta r_rendererPreference" without the quotation marks
It should say "Default"
Change it to "Shader Model 3.0"
Save.

Disable Shadows, performance improves drammaticaly. By the way why don't you want to disable Shadows?
