@ariamontgomeryy He was crazy, I think. Not like the other inmates, but the trauma of killing his wife and the loss of his kids pulled a wire. He refused to believe it happened, and he dwelled in the past. Seeing opportunity, the doctor tried curing him by playing along with his game of 'cops'. But it didn't work... so they basically decided to make him a vegetable to stop the violence.

At least that's what I understood.

The film is quite good, Scorsese isn't Kubrick :), the "Passacaglia" of Penderecki is amazing, perturbing, powerful: my personal thought's that the great film director Scorsese has used this piece in a forced way, the power of the music put at the beginnig of the movie overtop it, exceed the power of the movie intro.
