The chlorine in the water CAN, though I have to say rarely, kills pacman frogs. The chlorine can actually shorten their lifespan too. Leave it to set for 24 hours, and it should be fine.
Calcium is helpful, but no you don't need it. I don't use it :\
If you dust the crickets with vitamins it will help them live a longer life. The pacman's diet is very important to keep them healthy, mice are fine every once in a while but they are very fatty and not very nutritous.

Depending on which area you live, in the UK for instance, you need to use dechlorinator to put in chlorinated water to get rid of as many heavy metals and chemicals as possible. You can use tap water thats been sitting for a few days, but its best to still use dechlorinator.
Secondly, you really should dust your crickets or food items, though it depends what your feeding them to begin with.

yes i agree, bark and moss can lead to impaction if eaten. The best substrate is coconut husk fiber as it easily is passed through.
