There's no difficulty in the game. If you fiddle with a file in notepad under your Amnesia folder, you can have max health, sanity, oil, etc. He clearly at least had the health. The monsters weren't spawned - they both occur in this level (as well as another if a certain door is opened.) The first one was upon picking up a quest item in the back of the storage, the second one triggers from simply trying to exit.
