It's not from the original Rondo at all, but the basic idea comes from SNES Dracula X. If you didn't rescue Annette, she turned into a hybrid of Shaft and Carmilla (both absent in their true forms in the SNES remake) at the end of the clock tower level.
It wasn't in the original, but it was in Dracula X (SNES). If you didn't rescue Annette, she would transform into a - ...well, this. Sorta like Camilla.
