i think gta5 should be (i know this is fake, though i havent even watched it.) intertwined to all three cities (lc,vc,sa) with the ability to go to each one via the airport through the course of the game. the main character should be one YOU create, and eventually you will meet (through many odd jobs) the main characters from the previous GTAs, and through meeting them you meet NEW characters and while associating with them you find out whats been happening with them through all the prev. GTAs->

also the features (if not all) should be re-introduced (car modding, but more in-depth). you should be able to purchase bodyguard services (recalling VC's bodyguard reward at the end of the game, and picking up where SA's recruiting system left off) as well as being able to customize weapons (put them on your car if you really feel so inclined). also be able to park your vehicle ANYWHERE in the game (not too many, or theyll disappear as to not consume too much memory).
