I too think this song is about sex outside of a relationship, rather than a song about being transgendered. I think Amanda wrote it so that it has dual meaning, but the blatant obvious meaning is a sex change, where as the deeper meaning is the regret and depression that follows a one night stand, or casual sex. For example, "how you'd love to have a little thing with which to play" this is actually about a baby, as is this: " have to keep on feeding it tomorrow". Those are just a few examples. Find the lyrics and think about them more with this concept in mind.
